Forms for athletes with health needs

*If your child has asthma, a life threatening allergy, diabetes, or a seizure disorder, a healthcare plan must be submitted PRIOR to the start of the season*

Medication 

Medication should be given by the parent whenever possible.  When your child will require medication during a sports practice or game, a Medication Administration form MUST be submitted prior to the start of the sports season.  Please print this Medication Administration Form for any prescription or over the counter medication.
*The Medication Administration Form DOES NOT need to be completed for inhalers, epipens, insulin, or seizure medication listed on a healthcare plan.

Athletic Programs Advisory Committee

In accordance with Board of Education policy, the Cheyenne Mountain Athletic Programs Advisory Committee (the Committee) exists as a volunteer committee appointed by the Board with the responsibility of reviewing and evaluating District 12 Athletic Program finances, polices, procedures, rules, facilities, and other operational activities, and providing guidance and recommendations to the directors of athletics, principals, superintendent, and/or the Board of Education on such matters.
